SPECIAL  INSTRUCTIONS

TO  APPLY  FOR  A  H.O.G.^®  PA  REGISTRATION  PLATE

for

CARS  and  TRUCKS

 

 

Harley-Davidson Motor Company and the Pennsylvania Department of Motor Vehicles have established the following guidelines for PA HOG^® members who would like to purchase a special HOG^® PA registration license plate:

 

(1)        Applicant must have a current membership in National HOG^®.

(2)               Section C must not be filled in.

(3)        Mail the application to:

SPECIAL  INSTRUCTIONS:

 

Ø      Complete all parts of Section A

Ø      Complete all parts of Section B

Ø      Include your HOG^® National membership number in Section B, along with the name, address and phone number of your local Harley-Davidson dealership.

Ø      Do not fill in Section C

Ø      Applicant must sign and date Section D

Ø      Include payment of $ 30.00 in the form of a check or money order payable to:

PA HOG^® PLATE

Ø      Mail the application along with a check or money order for $ 30.00 to Jerry Longo at the above address.

Ø      Applications sent directly to the PA Department of Motor Vehicles will not be processed.

Note:      (1) Current membership in national HOG^® is mandatory to be eligible to receive and maintain the special HOG^® PA registration plate.

(2) $ 20.00 covers the $ 20.00 State registration fee plus $ 10.00 to cover HOG^® chapter expense for application, printing and mailing.
